Grant and Per Diem Social Work Coordinator

The incumbent is responsible for collecting and submitting GPD and ETH program participant data according to NEPEC and HOMES evaluation procedures and timelines. The incumbent coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices, and/or community agencies.The incumbent actively participates in the GPD and ETH program conference calls and attends training offered to homeless programs. The incumbent is responsible for following local policy and procedures for the GPD and ETH programs. This includes following policies and documentation requirements as required for CARF and Joint Commission accreditation. The incumbent takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. The incumbent performs accountability functions:
1) document all contacts with patients/families in the medical record; 2) completes statistical and other reports in accordance with VAPORHCS and respective professional discipline service policies. The incumbent is an active participant in the treatment planning process with other disciplines. The incumbent will provide consultation and education to Veterans and families on community resources, Advance Directives, and VA benefits. The incumbent will act as a consultant and social work expert for other team and staff members. The incumbent will provide a full range of advanced social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management, and other services as appropriate. The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for Veterans. The incumbent will participate in committee assignments, as deemed necessary by the service manager. The incumbent may participate in a rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends. The incumbent completes suicide risk screens and documents these assessments as required by policy. The Social Work Coordinator completes comprehensive assessments and collaborative safety plans when clinically indicated; provides follow-up appointments for all Veterans upon initiation on the High Risk for Potential Suicide List in compliance with VA policy; provides follow-up with Veterans who fail to attend appointments per local procedures; consults with the Suicide Prevention Coordinator, a member of the Suicide Risk Reduction Committee or a MHD supervisor as needed to ensure the safety and welfare all Veterans; and remains up-to-date on suicide prevention training materials and clinical tools, and utilizes as appropriate in clinical practice. Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education(CSWE) Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. //vaww.va.gov/OHRM/T38Hybrid/. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).NOTE:
If social workers covered under the grandfathering provision of the 1991 Federal law regarding licensure or certification of VHA social workers leave the GS-0185 social work series, they lose the grandfathering protection. If they choose to return at a later date to the GS-0185 series, they must be licensed or certified to qualify for employment as a social worker. Preferred
Experience:
2 years post LCSW with ability to provide LCSW supervision in Oregon or Washington state. Grade Determinations:
Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. References:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
